Q. How do I change my personal information?
A. Click "Mark as Incorrect" and Dental Care Center representative will contact you.
Q. How do I confirm an appointment?
A. Go into appointment details page by clicking the appointment, and press 'Confirm' button. If the appointment is already confirmed, you will see blue header.
Q. What to do if I forgot my username?
A. Email dentalcare@stonybrookmedicine.edu and you will receive a new one
Q. What to do if I forgot my password?
A. Go into Login page and click "Forgot Password"
Q. How do I change my password?
A. Click “Personal Information” on top panel to open the page, then click “Change Username and Password” on the left.
Q. How do I request a new appointment or reschedule an existing upcoming appointment?
A. Email your provider or patient manager in the “Messages” page or call us at 631 - 632 - 8989
Q. What should I do to prepare prior to my appointment on site?
A. Login to the SDM Patient Portal to check your personal information and insurance policy to see if they are accurate, update you Medical history and Dental history forms in the “iForm” section.
